Been around a while for HIV-I have seen the older model in use in Africa. It's a relatively stripped down qPCR test-a fair degree less accurate at about 98% than the current machines, but cheaper and more robust. Crucially does not need RNA isolation first. Actually it is quite a bit like the 1918 flu, if you think about it. In 1918 millions had just come out of 5 years of dreadful war and were emotionally and physically worn out. They had no modern medicines, not testing, no ICU units, no antibiotics, no anti-virals, not even penicillin. Very little oxygen, very few face masks or other protective gear.
