It looked like Janmaat clouted Montero in the neck and either Montero's hand went back and hit him or Janmaat just hit him and fell down to make it look like an accidental clash. Haven't seen any replays though.

The replays I saw on the day, made it look like montero came to a rest and Janmaat ran into him. the brace sent Jeff's head back and it hit with the Newcastle man's ivories.
