Well here is another one, a lack of staff meaning a cutting of service,

Then what happens to those who attend the MIU, they will be redirected to where? They'll probably end up at Morriston A&E putting further pressure on that already hard pressed unit.

Where will the Welsh NHS get these staff from, is the Senedd giving them more money?

I’m sure there are rubbish ones, Cat but my point is about numbers.

Private sector management runs at about 15% and the NHS At around 5%.

You can have as meany clinical staff as you like, but it’s not going to run well without decent coordination, administration, procurement, tech departments etc.

It amazes me that the systems are so antiquated that even something like accessing patient records in different sites seems so difficult 🤷🏻


The NHS does need massive reform but when the government tried that, it made a complete mess of it.

I'll accept your stats about managers but I think my point still stands, it doesn't matter if you have exactly the right amount of managers if they are rubbish.

My wife is always moaning about how hard it an be to get someone's records. Also, many GP surgeries still use a FAX and it is so slow and unwieldy.

My wife works for the NHS and she would disagree. The other problem is too many of these managers aren't promoted by ability so we end up with rubbish management as well as too many of them.

That applies to the very top too, we see people chosen by politicians to run trusts who aren't good enough. Money is spent on things that aren't needed. In Morriston the former female CEO, 2 CEO's back I think, spent thousands on LGBT things like a massive flag to fly at the front entrance. All money should be spent on what patients need, not minority causes.

Patients need good facilities, good staff and enough of them.

I’ve worked in a number of organisations where the workforce have been drawn from across a diverse spectrum including the LGBT community, the amount of hate and bigotry a large proportion of the staff have experienced is appalling. I can tell you that the staff do appreciate support from their managers and the wider organisation and flying the flag every now and again is welcomed by as a supportive act.

In the grand scheme of things purchasing a couple of flags is neither here nor there, there’s a lot of staff turnover in the NHS and any initiatives which will assist and improve staff retention should be welcomed.

I truly believe that part of the problem is the pandering, It's almost as if in the eyes of some people in power minorities are weaklings, too useless to form their own ideas or defend themselves if needed. That just pisses off those being belittled and those on the outside.

It's like the stories about Christmas being cancelled to avoid offending Moslems. I guarantee you that the vast majority of them couldn't give two shits one way or another, and it's being driven by do-gooders on their behalf. All it does is generate bad feeling for no reason whatsoever.

I certainly agree with you about as much as is possible about the systemic issues. We probably don't agree about how best to fix them.

For me, as long as NHS treatment remains free at the point of service then nothing else is off the table. I think as a society we could probably stand to spending a few more billion on it, but in return huge steps need to be made in delivering far more bang for that buck.

Having spent most of my working life seeing how the public sector does things, it would have to be a poorly run private enterprise indeed to manage processes worse.

I’ve said in here before that the NHS cannot carry on in its current role. I’d go a little further than you in as well as free at point of service, I see a national role for preventative work and support.

It just needs a strong government to have a go at it.

Daft thing is we can see what not to do (US) but there are other better examples of how to do it.

Singleton Hospital has sent senior professionals/clinicians from oncology dept out to Ireland by private charter flight(s) using twin engine propeller aircraft as opposed to more expensive commercial flights.

This is to save money, despite the fact that it's an extremely jittery and uncomfortable experience crossing the (all that water below!) rough Irish Sea in a smaller light aircraft like that. Talk about turbulence!!! Not thinking about it's employees psychological wellbeing in that particular incidence.

Then on the other hand you've got others in the public sector who'll (always have an excuse!) waste money on lesser priority things like the money does not belong to them.....Hang on mmmmm

I work on the basis that what people do in their own bedroom is of no interest to me. I am simply not interested. I would not discriminate because it is a subject of no relevance. I see no point in Pride marches or flags or promotion of a private lifestyle route.

Gwyn, I'm furious about EVERY penny I see as wasted, not just the mre expensive ideas.

Lets start at the top, the CEO gets a massive wage AND a company car on expenses, surely they can afford their own car. When they manage things so badly why do they deserve the penny AND the bun.
Also, for myself at least, if you want to be more appealing to LGBT then have a recruitment drive and spend the money targetting LGBT+ there, not on a flag.

Dr W, Roehm would probably have organised the Welsh NHS far better than our health sec but I'm not sure how far she'd go to protect her position

To tackle any back log in whatever industry you have to find ways of tackling the issue from both sides to prevent today's minor issues becoming tomorrow's severe problems and so reduce overall demand on the system.
There is some more movement in this direction with more diagnostic centres planned. I know what you are thinking, so what would you do. Well, the millions being spent on 20 mph road signs could have been used for cancer screening for example. In every industry I've worked in where there has been a production problem, the strategy has always been divert money from any activity that can be delayed or stopped to address problems before they escalate and drown the business.

I mentioned that stat earlier.

I have no doubt that there are poor managers and poor practices but there’s a real lack of numbers for such a monolith of an organisation.

What we’ve ended up with is pretty much a monopoly run on the good will of many.

It’s really easy to look at spending you don’t agree with and say it should be spent on doctors, nurses etc. but it’s rarely that simple.

It could be that the LGBT flags were paid for from an allocated budget heading for recruitment, diversity or equality for example.

Same in schools. When the inspectors come, we have to prove that diversity is woven through the curriculum and what we to to ensure there is support for children to feel comfortable whatever their characteristics.

I’m not trying to make a right or wrong point, just saying how it is. That makes it a central government issue, not a school, hospital or health board.

It's still a problem when so much money is diverted from the NHS raison d'etre.

Here's another issue, whistleblowers. They are supposed to be protected but I know a person who blew her whistle and instead of the problem she raised being addressed she was hounded out and offered a pay off as long as she signed an NDA.

Good staff are leaving because they'd rather get rid and shut them up. Lucy Letby is a very relevant example, 2 doctors who brought her up were told to apologise when she was murdering babies.
