Out of 150 trainee midwives completing their final year after many hours of unpaid work , stress , taking on new skills not to mention the thousands of pounds now in debt its annouced that theres only vacancies for 40 . Maybe one day one of the too many NHS managers could devise a sucession plan or would that be to hard ?

There is a workforce plan for the NHS, it's either crap or no one is paying attention to it.


NHS is run by to many admin staff and bed managers, my youngest was in hospital with a serious chest infection and she was sending him to a covid ward, I was in work at the time and a nurse that cares for him twice a week at home was with him luckily and phoned Mrs in work, bed manager made herself scarce when Mrs arrived with steam coming out of her ears.
He never went onto that ward in the end.

The whole NHS recruitment process is a mess. Artificial caps on the amount of training places for doctors, no guarantees of work for newly qualified paramedics despite huge ambulance waiting times, yet somehow we continue to ship trained professionals in from abroad.

The NHS will have to write off the students training bursary. Currently students have to work for the NHS for 2 years or they have to pay the bursary back. As thats now impossible those who wish to work abroad should be able to do so straight away. And if there is a staff shortage in a couple of years the NHS will be buggered.

Maybe the 'once for Wales' ethical international recruitment programme wasn't as well thought out as it could have been . A huge amount of effort has gone into it but no mention about how this will marry with the ongoing recruitment process of its own citizens.

Actually, it's not. The NHS has a far lower proportion of managers to staff than do most private companies.

Agree about the bed managers though, they put patients wherever there's a bed

Here are some links showing how commissioning of training places is supposed to work. Actually there have been problems in workforce planning right back to the days of the Welsh Office. HEIW seems no better,. Usually the number of training places has been held below what turned out to be required, at times leading to cuts in the university departments involved.

The joke of seeing a GP in Primary Care is a non event now , send a photo of the issue or arrange a phone call at any given time , so keep your phone handy .

Luckily I still have a few a basic knowledge of the faff that goes on to see a Doctor and even I have given up hope for an appointment .

The elderly with no idea how to use a smart phone and tinternet , they deserve better .

I’d be pretty surprised if the recruitment came from overseas. Most of the health boards brought in international nurses for a couple of years post pandemic as there were huge skills shortages and it takes 3 years for a student nurse to complete their training and graduate, the only way to fill the gaps was to bring in overseas nurses. In terms of senior clinicians, many health boards are still having difficulty in filling positions (registrars and consultant posts) and are still recruiting from abroad. Outside of the big cities and teaching hospitals it can be a real challenge.

There are comprehensive work force plans in place which are supposed to ensure that the number of staff coming through the education system matches the projected needs of the NHS (doctors is slightly different ) but there are lots of variables which will impact on the numbers.

One massive variable is finance. At the current time, Health Boards are having to make significant savings in operating costs. Health Boards delivered £253 million in savings in 2024–25, however the overall collective deficit was still £124 million and that figure has to be reduced or even eliminated during this financial year. When a massive proportion of of the NHS budget is wages, it’s clear to everyone that the easiest or potentially the only way to make the savings is to cut staff numbers.

There’s massive pressures from the Senedd to deliver these savings and deal with waiting lists, there’s pressure to reduce expenditure on agency nurses and doctors who are very expensive, cutting agency workers saves cash but it does mean gaps. Another thing that can be done is a reducing the number of nurses in duty at any one time, that increases the patient to nurse ratio, potentially increases the risk level but saves money.

Several nursing agencies in South West Wales have stopped trading because there’s less work available, that has resulted in agency nurses going back to work direct for the NHS which in turn means there are less jobs available for the students graduating.

In addition, nurses are delaying retirement because of costs and financial uncertainty, those that are retiring have the option of retire and rehire, basically they retire but can comeback and work part time for the NHS. That also reduces the number of jobs available, but it does mean a lot of highly skilled staff stay in the workplace.

The next streamline process (it allows student nurses graduating this year to apply for and select their jobs) has been delayed and will now go live in mid May - call me a cynic but I’d say it’s been deliberately pushed back to after the Senedd election.

Against this backdrop, we’ve got more people using NHS services, people living longer which means they cost more to care for, we have more social deprivation and poverty in Wales which is a huge determinant in health outcomes, people don’t want to pay tax for the services they get and a lot of people don’t put anything in at all and just take from the system.

" then be suprised because I'm not although the only suprise is I'm suprised that you are .NHS Wales actively recruits from overseas through the All_Wales International Recruitment Programme which since 2025 has appointed over 1,000 healthcare proffesionals .the majority being nurses .
So you have to ask yourself why then would the NHS take on over , for , example 150 trainee midwifes in 2023 only to be in aposition to offer 40 positions mand thats just the role of midwifes ,there are many more ward nurses completing their final year only to find they have no future working in the UK as a nurse .The Welsh NHS have a contract with the Government of Kerala in India to employ 200 nurses and doctors and yet many of our own recently qualified doctors can't find a post .

What nationality were the staff who carried out the operation. Its easy to say they only take jobs the British won't do but its not true. I have three neighbours not of British origin one a doctor, one a lecturer in the University and one works for Welsh Water who does something in the science line. All were the best person for the job not because British workers wouldn't do it or didn't apply. You can't just stereotype by what by what you think you see.

Dispite the now boring comments of " we need them to sell us vapes , cut our hair and sell us drugs" Care homes employ many immigrants at pittence wages yet charge an inmate 4k a month for the pleasure of attending to their needs,maybe the whole wage /benifits & taxation systems in this country should be stripped back to basics because its not working if you can get more money for sitting on your backside than to work .We dont have a industry in any capacity anymore just a service one which will never pay enough for peoples needs so instead of blaming the people maybe look at why China and other countries are doing better than us who used to be the powerhouse of the world.

I interviewed prospective employees over many years, you get an instinct as whether someone is suitable for a vacancy. There is a term "over qualified" which you have to factor in for many of the lower paid jobs with little chance of progression. Its not that you don't get the British applicants some will take any job when they lose their jobs but you know full well they will be gone when they get something better, so you don't employ them. You know how to spot a "sicker" who will take a lot longer before they leave, quite often they are non British and will find it difficult to get a better job. So its not always "British people are lazy and don't want to work" its horses for courses often employers employ to fit the position be it up the scale or down the scale, its not always lack of British candidates.
